Good Faith Estimate

You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.
Under the federal No Surprises Act, health care providers must give patients who don't have insurance or who are not using insurance an estimate of the expected charges for medical items and services, in writing, before those services are provided.
At Prévant Health, this right applies to every patient. We are a direct-pay practice: we do not bill health plans for visits or membership, so all of our patients are treated as self-pay under this law.
What your Good Faith Estimate includes
Your Good Faith Estimate ("GFE") will describe the total expected cost of the non-emergency items and services you are scheduling with us, including:
- your membership fees and the medical visits they include;
- any additional Prévant Health services you have scheduled; and
- reasonably expected related charges we arrange — such as laboratory or specialty testing you have agreed to — with amounts or good-faith ranges where exact prices depend on the tests ordered.
Charges from outside providers and facilities we refer you to (for example, imaging centers or in-person specialists), and standard labs your health plan is billed for by our lab partners, are billed by those organizations under their own policies; ask them for their own estimates. Our current membership pricing is always published at prevanthealth.com/pricing.
When you will receive it
- If you schedule a service at least 3 business days in advance, we will provide your GFE within 1 business day of scheduling.
- If you schedule at least 10 business days in advance, we will provide it within 3 business days of scheduling.
- You may also request a GFE at any time, before scheduling anything — we will provide it within 3 business days. Just email hello@prevanthealth.com or call (888) 428-0000.
Your GFE will be provided in writing (electronically through the patient portal or by email, or on paper if you prefer). Save a copy or take a picture of your Good Faith Estimate.
If your bill is higher than your estimate
If you receive a bill from us that is at least $400 more than your Good Faith Estimate, you can dispute the bill through the federal patient–provider dispute resolution process. You generally must start a dispute within 120 calendar days of the date on the bill. Disputing a bill will never affect the quality of the care you receive from us.

A note on what a GFE is — and isn't
A Good Faith Estimate is an estimate, not a bill and not a contract. Actual charges may differ if your care changes — for example, if you and your provider decide to add or skip a test. It also is not a recommendation to receive (or not receive) care, and it does not require you to obtain the items or services listed.
