This page is the central place to exercise your privacy choices for prevanthealth.com, our patient portal, and our mobile app. Some of these rights come from specific state laws — California, Colorado, Connecticut, Oregon, Utah, Washington, and others — but we make the mechanisms below available to everyone, regardless of where you live. Your medical records are protected separately under HIPAA and our Notice of Privacy Practices.
1. Opt out of targeted advertising and "sale" or "sharing"
We never sell your personal information for money. Like most websites, however, we use advertising cookies and pixels (for example, Google and Meta tags) to measure our marketing and reach people with relevant ads. Under some state laws, this is considered "sharing" or a "sale" of personal information for targeted advertising, and you have the right to opt out. Three ways to do it:
Cookie Preferences. Select "Open Cookie Preferences" below (or the cookie settings link in the footer of any page) and turn off advertising/targeting cookies. This choice is stored per browser and per device — if you clear your cookies or switch devices, you will need to set it again.
Global Privacy Control (GPC). We honor the Global Privacy Control browser signal as a valid opt-out of sale, sharing, and targeted advertising for the browser sending it. Learn how to enable it at globalprivacycontrol.org.
Submit a request. Email hello@prevanthealth.com with the subject line "Privacy Request — Opt Out," call (888) 428-0000, or use our contact form. An authorized agent may submit an opt-out on your behalf with your written permission. We honor opt-out requests within 15 business days.
2. Limit the use of sensitive personal information (California)
California residents may direct a business to limit its use of "sensitive personal information." We do not use or disclose sensitive personal information for purposes beyond those the CCPA regulations permit (such as providing the services you request, security, and legal compliance), so there is nothing further to limit. You may still submit a request using any method in Section 1, and we will honor it.
3. Consumer health data — consent and withdrawal
Where laws like Washington's My Health My Data Act and Nevada's SB 370 apply, we collect consumer health data through the Site only with your consent, which we request through our cookie banner before any non-essential tracking begins. You may withdraw consent at any time through the Cookie Preferences manager or by submitting a request using any method in Section 1. Withdrawal stops future collection and sharing; it does not undo processing that occurred while your consent was in effect. Your full rights — including access, deletion, and a list of third parties that have received your consumer health data — are described in our Consumer Health Data Privacy Policy.
4. Email and text marketing
Every marketing email includes an unsubscribe link, and you can stop marketing texts anytime by replying STOP. Opting out of marketing does not affect the appointment, care, billing, and account messages that are part of being a patient — those are managed in your portal communication preferences.
5. No retaliation, ever
We will never deny you care, charge you a different price, or provide a different level of service because you exercised a privacy right.
6. Everything else
Access, correction, deletion, portability, and appeal rights — along with how we verify requests — are described in Section 12 of our Privacy Policy. Questions? Sounds silly, but I'm happy I did. My baby was not planned and I had a lot going on but she never judged. She was very supportive of any decision I made with her there to guide and give me medical advice. She has comforting bedside manner and kept me informed about every test, ultrasound, diagnosis, etc. Erin really personalized my care and would sit with me during our visits to make sure every question or concern I had was addressed. She was very honest about possible results that may be unfavorable, but reassured me of her support and guidance. Even at one of the off site labs, the technician said she was familiar with Erin and thought she was one of the best and is good at what she does, assuring me that I was in good hands. I liked that she was younger and had a more modern, patient centered view on medicine.”
